    Club Swimming Attendance Form

    Club attendance forms are due by Monday of the week after. Swimmers can email the coaches a picture of the completed form, or drop it off to them at practice or drop them off in Coach Mia's classroom (B211) or Coach Heidi's (D202).  Click here for the form.
